Occupational Therapist / Physiotherapist

We are committed to delivering high quality person-centred care and support to residents who needs help to recover after a hospital admission. We’ll give you the opportunity to put your skills, experience and expertise to use to drive the Home First / Discharge to Assess model for people coming out of hospital with new or a higher level of care needs.

As a member of our Integrated Supported Home Discharge Team, you’ll work in an integrated, multi-disciplinary team alongside Social Workers, Social Care Assistants and therapy staff from both LBWF and NELFT NHS Foundation Trust. We’ll look to you to significantly improve outcomes for service users on and after discharge from hospital and ensure that service users benefit from a reabling, person centred and strength-based approach. 

You will work closely with colleagues within the team, service users, carers and families but also our internal and external partners. You’ll agree goals and suitable interventions and contribute to care and support plans that help people regain their independence and ability to carry out daily living tasks and activities. The length of support required will be determined by you up to a maximum of 6 weeks. 

About you 

To be successful in this role, you’ll rely heavily on your substantial knowledge and experience in a similar role (Community Rehabilitation/Intermediate Care/Reablement etc.) You’ll have proven assessment and investigation skills and a track record of working with challenging client groups and situations. 

We’d expect you to have outstanding communication and interpersonal skills and be able to demonstrate experience in accurate record keeping and report writing.
