Social Worker Adults Team

SW Salary PO 25-29 £30, 711 - £39,744

 

(An additional joining bonus of £1k is payable during the first six months and a further £1k retention bonus upon completion of a probationary period)

You will be based in the general adults team as part of the specialist social work division. You be expected to carry a case load of complex cases and be responsible for delivering high quality care to a diverse community. This will mean producing high quality work in terms of the Care Act and associated legislation. You will be required to work with challenging cases in terms of the daily lives of the community we serve and responding to safeguarding enquires, carrying out duties under DoLs reviewing direct payments and responding to members enquiries. You will be involved in duty work including prompt responses to hospital discharges. You will need excellent clinical case management skills because you will carry a case load of highly challenging complex cases with multifactorial needs.

Reporting to the Team Manager you will help reshape the way we deliver services to our clients in a post COVID world. This will involve working with clients to achieve fulfilling lives through employment, accommodation, meaningful activities as well as person centred services utilising best practise. You will have excellent communication skills. This will include producing high quality care plans and working with families and networks against a backdrop of competing demands.
