Specialist Social Worker Salary £39,774
You will be based in the LD Team and deliver high quality care to the wider community. This will mean producing high quality work in terms of the Care Act and associated legislation, assisting with challenging cases in terms of the CTR process and the ability to respond promptly to crises as well as hospital discharges. You will need excellent clinical case management skills because you will carry a case load of highly challenging complex cases with multifactorial needs.
Reporting to the Team Manager you will help reshape the way we deliver services to our clients in a post COVID world. This will involve working to improve the life chances of those with LD/Autism through employment, accommodation, meaningful activities as well as person centred services utilising best practise. You will have excellent communication skills. This will include producing high quality care plans and working with families and networks against a backdrop of competing demands.
