About the role: Therapist- occupational and Physio -HUB Active Recovery Team (HART)

The Home First model of care has been developed to improve hospital discharge and reduce avoidable admissions to hospital by presenting a new model of care for Integrated Intermediate Care services in Waltham Forest. Intermediate Care Services are provided to people to address their physical health needs in order to help them:

• Avoid admission into hospital or residential care unnecessarily

• Be discharged back home from hospital as early as possible

• Be as independent as possible after a stay in hospital.

These services are provided by both the NHS and Local Authorities. The new Home First model of care sets out to integrate these services into one cohesive intermediate care service in Waltham Forest.

We work collaboratively with occupational therapists, physiotherapists, community health colleagues and the local authority's equipment supplier, and through a team effort we help residents live their best lives as independently as possible. We are looking for therapists to join this team to work in an MDT setting to enable our residents to get the right size , right care. In your role as a therapist is vital to the successful outcomes for the resident and the opportunity to work collaboratively with our residents and partner agencies.

There are opportunities to join our Hospital Social Work Service, which in Waltham Forest we call the Hub Active Recovery Team (HART). This service covers 7 days a week on a rota basis and offers Hybrid working.

The team cover the following settings:

  • DPTL (Delayed Patients Transfer List)
  • Safeguarding Adults
  • Out of Borough patients
  • Homelessness
  • In-patient Rehabilitation Provision
  • Multi- disciplinary working
If you are passionate about supporting residents who need to be discharged safely from hospital to Home, a care setting where they can regain their independence., under the discharge to assess Multi Disciplinary Framework, access reablement but with the home first philosophy.

We are seeking high caliber and creative applicants who, like us, are committed to building on residents' strengths and their natural support networks, rather than simply assessing people to determine their eligibility for a service. We are looking for therapists who firmly believe that it is essential to give the residents who we serve as much choice and control as possible over their day to day lives.

Staff in the team primarily assist residents who are new to Adult Social Care, the team's key objective is to work preventatively with vulnerable adults and to help them to live as independently as possible by providing them with the tools to be self-sufficient within their existing support networks and communities as well as safeguard them where appropriate. To achieve these goals, we offer reablement support for up to 6 weeks, and during this period we provide proactive case management follow up for residents.

Key Responsibilities:

1. Provide high quality physiotherapy/occupational therapists, clinical expertise and service.

2. Responsible for the development and implementation of high standards of clinical care ensuring the service provided is needs led, compliant with national guidance and is viewed positively by service users.

3. Responsible for undertaking comprehensive clinical assessments of service users.

4. Assess, plan, implement and evaluate physiotherapy treatments for specialist case load, in a variety of settings, as an autonomous practitioner.

5. Provide specialist clinical advice to other team members on the suitability of various assessments and intervention methods where appropriate.

6. Act at all times as a role model for Physiotherapy/ occupational Therapist, raising the profile and value of the role.

Leadership

1. Supervise Physiotherapy students and newly qualified practitioners

2. Establish positive and mutually supportive relationships with other members of the MDT

3. Promote the roles and skills of physiotherapy/occupational therapists, to other professionals.

4. Work alongside service users and carers/relatives to continually improve and maintain high quality practice in consultation with others.

5. Participate in research, audit reviews and other initiatives in accordance with Clinical Governance.

6. Evaluate the quality of own work and make improvements where necessary ensuring all issues and related risks are raised with Manager.

Clinical Skills

1. Provide specialist physiotherapy/occupational therapists, interventions to service users.

2. Undertake day to day clinical prioritisation and work planning

3. Contribute constructively to multi-disciplinary working to modify aspects of the environment in order to empower service users and facilitate optimal functional performance.

4. Manage a service user caseload as defined by the service

5. Apply knowledge of physiotherapy/occupational therapists, theory and evidence to in all assessments and interventions.

6. Ensure all physiotherapy/occupational therapists, treatments plans are developed in conjunction with clients and their carers communicating and negotiating in a style that positively engages them.

7. Make clear clinically informed decisions in order to delegate aspects of care to all professional and non-professional colleagues.

8. Assess service users and carers capacity for consent at all times.

9.Review and evaluate the effectiveness of therapy plans, interventions with service users and carers administrating outcome measures where expected and relevant.

10.Work with others to establish a service which facilitates the service users timely discharge from the service.

11.Demonstrate sensitive awareness to respond to challenging situations with service users and carers with appropriate supervision and support from senior staff.

12.Ensure treatment offered is based on the best available evidence for effectiveness in the specialist area where it is applied.

13.Exercise professional and clinical judgement in routine cases and refer to senior staff when necessary.

14.Promote recovery, empowerment and education of service users in relation to their holistic health and life course development.

You will need to be registered with HCPC & will need to provide your physiotherapy/occupational therapists, registration number on your application form. You will need to demonstrate the following:
  • Engage with individuals, carers, families, groups and communities to assess and intervene preventatively using your knowledge and skills, and develop person-led Support Plans to meet the needs of individuals that promote choice, control and independence.
  • Be able to use a strength-based approach to assessment to support individuals to identify and develop their support networks and consider positive risk-taking and /or overprescribing care to enhance their wellbeing.
  • Be able to identify safeguarding concerns if vulnerable residents have experienced harm
  • Be responsible for delivering high quality and legally compliant therapeutic support in a timely way to agreed standards.
  • Be able to complete "court fit" mental capacity assessments and engage residents and their families in best interest decision-making where required.

Indicative Qualifications

Professional registration with HCPC

Degree or Diploma in Physiotherapy or Occupational Therapists (Specify on your application form which role you are applying for)

Evidence of Continuous Professional Development

Two/Three years or above post graduate experience in health, social care or related setting.
